Recovery at Home
We empower patients to successfully recover at home, when their condition does not require in-patient care. Our software and service platform tracks critical measures of patient recovery, establishes links to home and community based services and provides assessments to determine the services required to achieve successful recovery in the home setting.
Local Performance Networks
We develop local networks of organizations who can serve patients after their surgery or hospitalization. This service covers home health, skilled nursing facilities and other post-acute providers.
Direct Patient Support with the Care Innovation Center
Remedy Care Innovation Center (CIC) provides telephonic support to patients during recovery at home.
The CIC concentrates on supporting patients in the following areas:
- Medication dosage, type and proper usage
- Follow up with primary care physician or Specialist
- Accessible transportation to appointments
- HHA agency appointments
- Other issues (i.e lack of food, issues paying for utilities, etc.)
